- Meet with the senior and his/her family at home or at the hospital in order to assess his/her needs.
- Work alongside professionals in the health and social services network.
- Search for and recommend certified private residences that will meet the senior's needs.
- Inform the senior and his/her family about the cost of available spots.
- Visit selected private residences (CHSLD, semi-autonomous, Alzheimer, etc.)
- Inform the senior and his/her family about the policies relating to leases and the steps to take in case of incapacity (incapacity mandate, power of attorney, lease termination, etc.)
- Inform the senior and his/her family about government financial assistance programs and tax credits, etc
- Follow-up with the senior after having moved in.
- Assess the quality of care and services within the residence
- Refer the senior to another resource if private seniors' housing isn't the best option for their situation.
